Collection Instructions for Pharmacogenetic Testing

Complete & print requisition form online or fill out paper requisition form. Make copies (both sides) of patient license & insurance card.

Print out patient label for tube or fill out patient label provided in kit. Name on patient label should match requisition form.

Wash hands. Open the swab package ensuring the sponge tip does not come in contact with any surface prior to collection.

Place sponge in the mouth & rub along the lower gum in a back and forth motion 10 times.

Repeat step 1 on the other side of mouth.

Holding the tube upright to prevent spilling, unscrew the blue cap from the tube without touching the sponge tip.

Turn the cap upside down. Without spilling, insert the sponge into the tube & close cap tightly.

Invert the capped tube and shake vigorously 10 times. Place patient label on tube.

Place tube inside biohazard bag & seal bag.

Place requisition form & patient forms in pocket on outside of biohazard bag.

Place biohazard bag inside white mailer & seal. Place white mailer inside pre-paid return labpak & seal.
